Formal notice of pregnancy
I am writing to formally notify you of my pregnancy and my intentions for maternity leave.

My expected due date is: 6th August 2008
I intend to start my maternity leave on Monday 28th July – this will be immediately preceded by 3 weeks holiday entitlement.
It is my intention to return to work during February 2009 on a full time basis.
I do not yet have my Mat1B form but once received I will forward the original for your records, retaining a copy for myself.
I would be grateful if you could arrange for me to have a meeting where I can be briefed on the impact to my salary during maternity leave.
I would also like more information about the ‘Keep in touch#%92 days that are mentioned on the government website as I feel these may be beneficial to ease my transition back into work during the latter part of my maternity leave.
