Ok, spoke to hubby. He said as its Cyprus its not worth getting the wording from one of the packs, etc. from WH Smiths or online. You need to make sure this is properly completed otherwise they may not marry you!!
He suggests you call a couple of local law firms and ask how much they charge. It shouldnt be much as the document is probably only going to be a page long. Once it is drawn up you will then need to "swear" it, ie sign it and have it witnessed. You will need to go to another firm of solicitors to complete the swear.
